DFR0140 overvoltage?!

My DFR0140 is passing the input voltage straight through to the nominal 5V output pins. I've been powering it from a 9+V power supply, resulting in overvoltage on downstream components that expect 5V.

Any ideas what might have happened to the voltage regulator? Is this something I did, or a defective part?

Momentary over-voltage on the components may makes hot, if it was kept on, then it will be burnt out. Leff
