From the wiring diagram: this board connects two ways.
The encoder/sensor side is I2C - 4 wires (5V, GND, SCL, SDA) running to the shield's I2C port; the sensor connector itself is GH1.25-4Pin (from the spec table).The motor side is the three-phase UVW leads (3 wires) that go into the motor driver.
